Roles and Responsibilities
One of the keys to a successful Longitudinal Medical Record (LMR) implementation is creating a project team with clearly defined roles and responsibilities. Members of high performance teams understand their own responsibilities as well as those of their team members. In implementing the LMR, clearly defined roles and responsibilities will help ensure coordination and effectiveness. In this chapter, these roles and responsibilities are defined and outlined. The LMR Project Team members include:

Implementation Analysts
Physician Champion
Practice Super-User
LMR Support Analyst
Support Help Desk
Hardware Resource
EMR Adoption Consultant


Each of these roles will participate in the planning, implementation, and maintenance of the LMR in your practice. Together, these roles will be responsible to:

Guide the overall planning, strategy, and implementation of the LMR at your practice
Provide multi-disciplinary expertise and advice on workflow, application customization, and staff training
Create a detailed implementation plan for your practice
Ensure that key project milestones are met and that the project stays on budget and on schedule
Identify and define goals for Go-Live
Assist with the creation and maintenance of LMR custom clinical content
Integrate practice schedule, registration, and clinical data into the LMR
Support the LMR application and practice interfaces after Go-Live
Meet with practice staff to communicate the project plan and progress
