The Longitudinal Medical Record (LMR) is the ambulatory electronic medical record system used across Partners Healthcare System.
With LMR, providers can maintain patient records electronically and manage workflow with ease. The structured patient clinical data and tools such as charting, result management, referral management, and order entry help reduce paper, enhance data access, and reduce time in non direct care related activities. As a result, providers will be able to spend more time with patients and improve quality of life. In addition, providers can easily share data with other clinicians across Partners to facilitate communication and ensure continuity of care.
Developed in house, LMR represents a key component of the Partners information system strategy. World-class medical informaticians and researchers at Partners are closely involved in the product design and development.
This Complete EHR is 2014 Edition compliant and has been certified by an ONC-ACB in accordance with the applicable certification criteria adopted by the Secretary of Health and Human Services. This certification does not represent an endorsement by the U.S. Department of Health and Human Services.

For access to LMR over the Internet, please contact the LMR team by submitting a request through User Feedback, located under the Resource drop-down menu, in the LMR Web application.
Deactivate a User
To remove a user's access to LMR, please contact the LMR team by submitting a request through User Feedback, located under the Resource drop-down menu, in the LMR Web application.
Login Instructions
You can access the application from any computer with a high speed internet connection. On your first log-in, use your current User ID and password. You will then be prompted to change your password for future log-ins.
You will now have two passwords - one to login to LMR through LMR over the Internet, and another to sign while in the LMR application. Whenever you change your LMR over the Internet password, you will be changing your password to "login" to LMR through LMR over the Internet only. You will not be able to change your password (key) to "sign" while in the LMR.
